Pennsylvania LIBC-767: IRE Determination Face Sheet

After an impairment rating evaluation (IRE) under Section 306(a.3) of the Pennsylvania Workers' Compensation Act, the examining physician reports the result on this face sheet. It states the whole-person impairment percentage, attaches the report prepared under the AMA Guides, 6th edition, and certifies the physician's qualifications. The insurer can then use the rating, through its own Notice of Change of Disability Status (LIBC-764), to address the employee's disability status.

Who completes it

The examining physician completes the face sheet, including the attestation, impairment percentage, exam date and charge. Page one carries claim party details for the employee, attorneys, employer, insurer and claim representative.

Submitting and payment

How to submit it
Bureau notification happens when the physician completes the electronic LIBC-767 in WCAIS; the report also goes to the employee, the employee's attorney if known, and the insurer within 30 days of the evaluation.
Who pays
The physician states the evaluation charge on the form, and it is billed to the insurer, or to the third-party administrator if the employer is self-insured.

Good to know

  • The physician attests to being licensed in Pennsylvania, certified by an ABMS-approved board or its osteopathic equivalent, and in active clinical practice at least 20 hours a week.
  • Ratings use the AMA Guides to the Evaluation of Permanent Impairment, 6th edition, second printing April 2009, as stated on the face sheet.
  • A fillable PDF exists, but the face sheet itself says Bureau notification is done through the electronic version in WCAIS.
  • The rating defines the degree of impairment due to the compensable injury only, if any, which is the question Section 306(a.3) asks.
