Ohio BWC ECP-TX: Physician's Treatment Request

Physicians treating Ohio state-fund claims use this form to send the managed care organization (MCO) a comprehensive treatment plan, recommend additional conditions, or report a changed diagnosis. It is used even when services fall under the 60-day presumptive authorization. The MCO records its decision in Section V, while BWC rules on any recommended additional conditions and notifies all parties.

Who completes it

The physician of record (POR) completes Sections I to III and must sign with an individual BWC provider number; Section IV names the treating physician, such as a surgeon, if different from the POR. The MCO completes Section V.

Submitting and payment

How to submit it
Mail or fax the ECP-TX to the injured worker's MCO; the form cannot be used for claims of self-insuring employers.
Who pays
For an initial care plan in an allowed claim, BWC pays specialists and ancillary providers for services that follow the Official Disability Guidelines before MCO authorization, while treatment outside ODG needs MCO prior authorization.

Good to know

  • Section I needs the treating diagnosis with body part and levels, begin and end dates, CPT or HCPCS codes, frequency, duration, site-of-service code and rendering provider.
  • Mark whether the plan is an initial care plan or a subsequent care plan; the form says incomplete sections can delay processing.
  • Recommending an additional condition requires supporting medical documentation, a narrative diagnosis with location and site, and an explained opinion on whether it is causally related to the injury.
  • If the MCO marks the request pending, the requested documentation must reach the MCO case manager within 10 business days or the request may be denied.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can the ECP-TX be used for a self-insuring employer?

No. The instructions state that the form cannot be used for claims of self-insuring employers. It is for injured workers employed by state-fund employers, and the completed request goes by mail or fax to the managed care organization assigned to the claim.

What does approved with disclaimer mean on the ECP-TX?

The MCO uses it when BWC or the Industrial Commission is still considering the claim for initial allowance on the date the MCO signs. If the claim is ultimately disallowed, BWC may not cover the authorized services or supplies, and the injured worker may be responsible for them.
