Ohio BWC C-196: Amputation and Loss of Use Diagram

Ohio BWC uses this two-page diagram to set scheduled-loss awards for fingers and thumbs under ORC 4123.57(B). The physician marks the exact point of amputation and any ankylosed joints on dorsal views of the right and left hand, initials each mark and signs a certification. BWC staff then apply the loss schedule printed in the box reserved for BWC use.

Who completes it

The physician completes the markings, initials them and signs and dates the certification on the page for the affected hand. The injured worker's name and claim number go at the top; the weeks schedule is for BWC use only.

Submitting and payment

How to submit it
Return the signed diagram to BWC with the claim documentation for the loss award; the form itself lists no filing address.
Who pays
The C-196 sets no fee; the weeks of compensation it supports are calculated by BWC from the markings.

Good to know

  • Mark an amputation with a single line at the exact level, and show ankylosis by circling the affected joint; every marking must be initialed.
  • Per the diagram notes, losing the distal phalanx equals one third of a finger, the middle phalanx two thirds, and more than both the whole finger.
  • For the thumb, loss of the distal phalanx equals half the thumb, and ankylosis at the thumb IP joint counts as half loss of use under State ex rel. Riter.
  • The PDF is not fillable, so the diagrams are printed and marked by hand before the form is returned.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How do I mark ankylosis on the C-196?

Circle the affected joint on the hand diagram rather than drawing a line, and initial the marking. Amputations are shown differently, with a single line at the exact point of amputation. Use the right-hand page or the left-hand page depending on the injured side, then sign and date the certification on that page.

Does the C-196 cover toes or feet?

The current C-196 (BWC-1326, Rev. Feb. 19, 2026) shows only the posterior (dorsal) view of the right hand on page one and the left hand on page two. Its notes and weeks schedule address the thumb and fingers, including the extra weeks added for loss of a metacarpal bone.
