NC Form 25R: Evaluation for Permanent Impairment

Once a North Carolina workers' compensation patient's condition has stabilized, the treating or rating physician records the permanent impairment rating on Form 25R. Ratings are entered as a percentage for each member, from individual fingers and toes to the hand, arm, foot, leg and back, supporting the carrier's scheduled-loss compensation. The form states that its use is required under the Workers' Compensation Act.

Who completes it

The rating physician completes the whole form, including impairment percentages, the MMI and restriction answers, and any teeth, vision, hearing or disfigurement findings, then signs with printed name and federal tax ID. Employee, employer and carrier details at the top identify the claim.

Submitting and payment

How to submit it
The medical provider returns the completed form to the insurance carrier and must give a copy to the employee or the employee's attorney of record. Carriers file it with the Commission through its Electronic Document Filing Portal.

Good to know

  • For each rated body part the form asks whether the employee is at maximum medical improvement and whether the employee was released with restrictions.
  • Amputations are described on the reverse. Hearing ratings need the scale used, percentage loss per ear, and attached audiograms with the hearing loss calculations.
  • Vision is reported as distance and near readings without a corrective lens, and dental findings list crowns and extractions by tooth number with the employee's age.
  • The Commission's Rating Guide says ratings refer to the back rather than the spine and are given at the end of the healing period; the final rating is the examining doctor's independent opinion.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who completes NC Form 25R?

The treating or rating physician who evaluates the employee's permanent impairment. The physician enters a percentage of impairment for each affected member, answers the MMI and restriction questions for the rated body part, adds comments if needed, and signs. The form is returned to the carrier, with a copy to the employee or attorney.

How is impairment rated on Form 25R?

North Carolina uses the Industrial Commission's Rating Guide, which notes that AMA whole-person ratings cannot be readily translated to its system. The physician's job is only to state what percentage of a member or system is impaired; the Commission converts that percentage into an award of compensation weeks.

Does Form 25R cover hearing and vision loss?

Yes. It has a hearing section, with the scale used and percentage loss for each ear plus attached audiograms and calculations, and a vision section recorded without corrective lenses at distance and near. It also covers teeth, disfigurement of the face, head or body, and permanent impairment of any other organ or body part.
