Part D IRE Reconsideration Request Form (Level 2)

After a Part D plan upholds a drug denial at redetermination, the enrollee, a representative or the prescriber can ask the Independent Review Entity to reconsider, and this CMS model form is the written request. The IRE, C2C Innovative Solutions, reviews the case outside the plan. Plans include the form with each adverse redetermination notice.

Who completes it

The enrollee, a representative or the prescriber completes it, and the prescriber may file without an appointment. The prescriber part covers prescriber information and a supporting statement, which matters most for expedited and exception requests.

Submitting and payment

How to submit it
File in writing (fax counts) by mail, fax or C2C's online appellant portal, within 65 calendar days of the plan's redetermination notice.
Who pays
For payment requests, the IRE must issue a written decision, and instruct the plan to pay when appropriate, within 14 calendar days of receiving the request.

Good to know

  • The IRE decides within 72 hours for expedited requests and 7 calendar days for standard ones, counted from receipt of the request.
  • For exception requests, the IRE can extend its timeframe by up to 14 calendar days if the prescriber's supporting statement has not arrived.
  • An unfavorable IRE decision notice explains how to request a hearing before an Administrative Law Judge at the Office of Medicare Hearings and Appeals.
  • The current version took effect January 1, 2025. CMS posts it as a ZIP download, Word and PDF, on its Part D appeals forms page.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who handles Medicare Part D reconsiderations?

C2C Innovative Solutions, Inc., the Part D Independent Review Entity, also called the Part D Qualified Independent Contractor. Requests reach it in writing by mail, fax or its online portal, not through the drug plan.

How long do I have to request a Part D reconsideration?

Within 65 calendar days from the date of the plan sponsor's redetermination decision notice. The IRE then decides within 72 hours for expedited requests or 7 calendar days for standard requests, and within 14 calendar days for payment requests.

Can a prescriber request IRE reconsideration?

Yes. An enrollee, the enrollee's representative or the enrollee's prescriber may request standard or expedited reconsideration after an unfavorable or partially favorable redetermination. For exception requests the prescriber's supporting statement is needed, and its absence can extend the decision timeframe by up to 14 days.
