New York Life 25034: Mental Health Physician Statement

For an individual disability claim based on a psychiatric condition, New York Life sends the treating clinician this two-page statement instead of a general physician form. It asks for a DSM-5 formulation with supporting data, a structured mental status summary, how the condition affects work, home and social functioning, current psychotropic medications and psychotherapy, and how return to work is being addressed in the treatment plan.

Who completes it

New York Life completes Step 1 with the patient and policy details. The attending physician or treating mental health clinician completes Step 2, Sections 1 to 3, and signs the Step 3 declaration.

Submitting and payment

How to submit it
Step 4 lists mail, fax and email to New York Life Individual Disability Insurance.
Who pays
The form sets no fee and does not say who pays for the statement.

Good to know

  • The mental status grid covers appearance, speech, psychomotor slowing or agitation, reported and observed mood, anxiety, thought process, psychotic symptoms, judgment, insight, appetite, sleep and cognitive function.
  • Section 2 asks whether treatment compliance is an issue, whether drug or alcohol use is a factor, and whether the patient is in a recovery program.
  • Section 3 asks whether the clinician would support a full-time return if the patient arrived at the next visit with an appropriate job offer, and whether the patient can handle funds.
  • Edition 07/2022. It pairs with the Physician's Statement for New Claim, form 25027, when the disabling condition is psychiatric.
