Who completes it

The employer or plan administrator completes Part I and the employee completes Part II and the authorization. The attending physician completes and signs Part III, the Attending Physician's Statement.

Submitting and payment

How to submit it
Return the whole claim, with the enrollment form and any beneficiary changes, to United of Omaha's Group Life Claims by mail or fax.
Who pays
Part III states it should be completed without expense to United of Omaha Life Insurance Company.

Good to know

  • Part III asks whether the patient is totally disabled from the present occupation, and separately from any and all occupations.
  • Restrictions (what the patient should not do) and limitations (what the patient cannot do) are entered separately.
  • For a cardiac condition, give functional capacity Class 1 to 4. Expected change in condition ranges from 1 to 2 months to more than 1.5 years.
  • The physician signature line is marked no stamp. Mutual of Omaha notes further documentation might be needed later in the claim process.
