Mutual of Omaha M20339: LTC Attending Physician Form

Mutual of Omaha uses this two-page physician form when a long-term care insurance policyholder applies for benefits. The attending physician documents principal diagnoses, any organic mental impairment and how it was diagnosed, which activities of daily living the patient cannot perform without human assistance, services in place, the recommended plan of care and the expected course of functional status.

Who completes it

The attending physician completes the whole form and signs it with tax ID. The claimant or legal representative completes the separate Application for Long Term Care Benefits (M20339PO) and the authorization (M23333).

Submitting and payment

How to submit it
The completed form goes back to Mutual of Omaha with the claimant's Application for Long Term Care Benefits.

Good to know

  • For dependency due to organic mental impairment such as Alzheimer's disease, give the type of mental status testing used and the score.
  • For each of eating, toileting, transferring, bathing, dressing and continence, record the date the capability ended and the expected recovery date.
  • If the patient is in an institution, state whether home care is possible; if at home, check the skilled, support, family or equipment services in place.
  • The form, revision 3-2002, still asks for ICD-9 codes. Mutual of Omaha lists it as the Attending Physician Statement under Long-Term Care Forms.
