Lincoln GLC-01817ATL: STD Attending Physician Statement

For a Lincoln Financial group short-term disability claim, the attending physician answers a single page of numbered questions. They cover symptom onset, the date the patient stopped work, prior similar conditions, work relatedness, diagnosis, pregnancy due date, findings and treatment, progress, confinement, dates of total disability, and whether the patient is totally disabled from their present occupation or from all occupations.

Who completes it

The attending physician answers questions 1 to 27, including the patient's name, birth date and policy number, and signs personally, since stamps are not accepted. There is no patient or employer section.

Submitting and payment

How to submit it
Lincoln sends the form as part of each claim, and the physician faxes the completed page back to Lincoln's group disability claims office.
Who pays
The form sets no fee and does not say who pays for completing it.

Good to know

  • The form prints Lincoln's definition: total disability means inability to perform the material duties of the regular occupation, and anyone working for wage or profit is not totally disabled.
  • Partial disability means being unable to perform one or more material and substantial duties of the regular occupation, or unable to perform them full-time.
  • Question 20 asks when trial employment could begin, full-time or part-time, and when the patient will recover enough to perform their duties, in days, weeks or months.
  • Lincoln does not post this form publicly; the copy shown is the 3/10 edition, so use the version Lincoln sends with the claim. Companion LTD form GLC-01450ATL.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
