Guardian NJTDBDS1: New Jersey TDB Claim, Part B

Guardian uses this six-page packet for New Jersey temporary disability benefit claims under an employer's Guardian private plan. It follows the state's TDI claim layout: Part A for the claimant, a Part B medical certificate for the doctor, and Part C for the employer. The claimant must file within 30 days after the disability starts, and late filing can lead to denial or reduction.

Who completes it

The employer completes Part C and returns it to the employee, who completes Parts A and A1. The employee then gives the form to the treating provider, who completes Part B after the disability begins.

Submitting and payment

How to submit it
Guardian recommends sending all sections together to its State Disability Claims unit by mail, fax or email, since separate submissions could delay the claim.
Who pays
No fee may be charged: Part B cites 12:18-1.6, which prohibits charging a fee to complete the form.

Good to know

  • Part B asks for the care period, treatment frequency, last visit, the date the patient could not do regular work, estimated recovery date, diagnosis with ICD code, and supporting clinical data and tests.
  • For pregnancy, give the estimated delivery date and complications, and, if the pregnancy ended, the date and reason: birth, C-section, miscarriage or abortion.
  • The doctor answers whether surgery is cosmetic only, whether the disability arose from the nature of the work, and whether the patient can handle their own affairs, including benefits.
  • Guardian stresses that every section must be complete, accurate and legible, because missing, incomplete or illegible information could delay payment of the claim.
