Guardian GG-117: Waiver of Premium Physician Statement

A Guardian group life insured who becomes totally disabled can apply to have premiums waived, and GG-117 is the attending physician's statement for that request. The physician documents ICD-10 diagnoses, condition history, treatment program, progress and medical stability, whether the patient can do any part-time or full-time sedentary or light work, physical capacities, cardiac and psychiatric impairment, and competency.

Who completes it

The employee completes the Employee Section and signs the authorization. The attending physician completes the Physician Section, items 1 to 33, and signs with degree, specialty and tax ID.

Submitting and payment

How to submit it
The completed statement goes to Guardian Group Life Claims by mail or fax, or through the Secure Channel on Guardian Anytime.
Who pays
The Physician Section states that the patient is responsible for the cost of completing the form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
