Guardian GG-011711: Accelerated Benefit Rider APS

A Guardian group life insured with a terminal illness can apply for part of the death benefit early under the accelerated benefit rider, and GG-011711 is the attending physician's statement for that claim. The physician gives the treatment history, mental competency, symptoms and objective findings, current confinement, a detailed diagnosis, treatment with medications, and a prognosis of how long the patient is expected to live.

Who completes it

The attending physician completes the whole statement, including patient information, and signs with degree or specialty. The insured applies separately on the Accelerated Group Life Benefit Application, GG-012406.

Submitting and payment

How to submit it
The physician sends the completed statement to Guardian Group Life Claims by mail or fax, or uploads it through the Secure Channel on Guardian Anytime.
Who pays
The form states the physician is responsible for completing it without expense to Guardian.

Good to know

  • Guardian fills in the plan's qualifying limit at the top: the insured's life expectancy must not exceed a stated number of months to qualify.
  • The Remarks box asks the physician to explain if the life expectancy estimate is based on the current course of treatment.
  • The physician states whether the patient is mentally competent, meaning they understand their actions and can handle their own affairs.
  • Edition 12/17. A group universal life variant, GG-011711-GUL, is also listed by Guardian.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long must life expectancy be for Guardian's accelerated benefit?

The limit depends on the plan. Guardian writes the number of months into the blank at the top of GG-011711, and the insured's life expectancy must not exceed it. The physician answers the prognosis question, how long the patient is expected to live, with that limit in view.

What patient status does GG-011711 record?

The physician marks whether the patient is ambulatory, house-confined, bed-confined, hospital-confined or in a nursing home, and names the institution if confined. The form also asks for first, last and next visit dates and how often the patient is seen.
