Family Heritage A8CLM-ST2: Accident Physician Statement

Family Heritage Life, a Globe Life company, uses this one-page supplemental physician's statement as side 2 of its accident policy claim. The treating physician documents the diagnosis and codes, the accident date and nature of the injury, hospitalization and CPT procedure codes. A separate block on the same page covers intensive care claims, including ICU confinement dates.

Who completes it

The policyholder completes side 1, the accident claim form. The treating physician completes and signs side 2, and whoever fills it out prints their name and position.

Submitting and payment

How to submit it
Return side 2 with the policyholder's claim form to the Family Heritage claims department. Globe Life accepts these claims through its eClaims portal, which takes uploaded documents, or by mail or fax.

Good to know

  • The physician states whether the patient was under the influence of an intoxicant or narcotic at the time of the accident, whether it was physician-directed, and whether it contributed to the injury.
  • Item 9 asks for any other disease or infirmity affecting the present condition, and item 12 whether hospitalization was solely due to this condition.
  • The intensive care block asks about any heart attack, heart disease or stroke history, first diagnosis and treatment dates, ICU confinement dates, and any AIDS or ARC diagnosis.
  • Globe Life says claims are reviewed once all required documents arrive, and explanation of benefits forms from other insurers are not accepted.
