MetLife PFML Certification Form (All Leaves)

MetLife uses one certification for the state paid family and medical leave claims it administers or insures, covering the employee's own serious health condition, care of a family member, bonding, military leave, safe leave and organ or bone marrow donation. The employee completes Section 1, and the treating provider completes Section 2, certifying the condition, treatment and hospital dates, and the continuous or intermittent absence needed.

Who completes it

The treating health care provider completes Section 2 and signs it with license number and state. The employee completes Section 1 and, when they apply, Sections 3 to 5 on bonding, military and safe leave.

Submitting and payment

How to submit it
Mail or fax it to MetLife Disability as listed in Section 6, with the employee's name and claim number on every page.

Good to know

  • Durations marked lifetime, unknown, as needed or indeterminate may be returned as incomplete, so give start and end dates plus the frequency and length of episodes.
  • Section 2 asks whether the condition prevents the material and substantial duties of the patient's job, and covers pregnancy, due date, birth details and organ or bone marrow donation.
  • For family care, describe the care needed and why it is medically necessary; for an adult child, list the ADLs or IADLs that need support.
  • Qualifying reasons and covered family members vary by state and program, and the safe leave section refers to Connecticut's paid leave program.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who completes Section 2 of the MetLife PFML Certification Form?

The treating health care provider of the patient, who may be the employee or the employee's family member. The provider certifies the serious health condition, treatment and hospital dates, and expected absences, then signs with name, license number, state and business name.

Does a provider need to complete the form for bonding leave?

Not always. For bonding, Section 3 lets the employee supply a copy of the birth certificate or adoption or foster placement documents instead, with the healthcare provider certification in Section 2 as one of the listed options.

Why would MetLife return the certification?

MetLife warns that forms marked lifetime, unknown, as needed, indeterminate or similar may be returned as incomplete. The employee's name and claim number are needed on every page so MetLife can match the certification to the absence request, and intermittent leave needs a frequency and episode length.
