Washington DOH 422-065: Consulting Provider Compliance

A consulting qualified medical provider must examine a patient who requests medication under Washington's Death with Dignity Act and review the records to confirm eligibility. This one-page DOH form captures that consultation: the exam date, the six determinations both providers must make, the information given to the patient, and whether a psychiatric or psychological referral was needed.

Who completes it

The consulting qualified medical provider completes and signs the whole form, including their contact details in Section E. The attending provider is identified in Section B and receives the finished form.

Submitting and payment

How to submit it
Return it to the attending qualified medical provider immediately after the examination; do not send it to DOH directly. The attending provider files it with DOH within 30 calendar days of writing the prescription.
Who pays
The DOH form and instructions set no fee or billing code for this report.

Good to know

  • The six determinations are terminal disease, six months or less to live, competence, Washington residency, acting voluntarily, and a decision made after being fully informed.
  • Section D requires exactly one choice: you found no psychiatric or psychological disorder or depression causing impaired judgment, or you referred the patient and name that provider.
  • Disclosures to check include the diagnosis, prognosis, potential risks and result of the medication, and feasible alternatives such as comfort care, hospice care and pain control.
  • The July 2023 edition asks you to fill in all text fields and check every box to show each task was completed.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the consulting provider send DOH 422-065 to the Department of Health?

No. The instructions say to complete and return the form to the attending qualified medical provider immediately after the examination and not to send it to DOH directly. The attending provider attaches it to the Attending Qualified Medical Provider Compliance Form (DOH 422-064) and submits both within 30 days of prescribing.

What does the consulting provider confirm on DOH 422-065?

That the patient has a terminal disease with six months or less to live, is competent, is a Washington resident, is acting voluntarily, and decided after being fully informed. Both the attending and consulting providers must make these determinations, and the consultant also records whether a mental health evaluation referral was made.
