SEPTA Access Vision Disability Verification Form

SEPTA Access, the ADA paratransit service for Philadelphia and the surrounding counties, asks applicants with a visual disability to have an eye doctor complete the Vision Disability Only page. The doctor reports corrected visual acuity, visual fields and the functional effect of the vision loss. The page goes back to SEPTA Access with the ADA application and feeds into the eligibility decision.

Who completes it

An ophthalmologist, optometrist or neuro-ophthalmologist completes the whole page. The applicant completes the ADA application and the release of information in the same packet.

Submitting and payment

How to submit it
The completed page is returned with the ADA application to the SEPTA Access office in Philadelphia.

Good to know

  • The page appears as the last page of SEPTA's ADA Application Packet and also in a separate ADA Paratransit Service Vision Disability Application.
  • Applicants are pre-screened by phone with SEPTA Access customer service, which mails the application, or they print it from SEPTA's website.
  • After the application is returned, SEPTA Access schedules a functional assessment and disability interview run by licensed professionals, and decides eligibility within 21 days of receiving the application.
  • SEPTA Access was named CCT Connect until July 1, 2024, so older paperwork or patients may still use the former name.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete the SEPTA Access vision form?

The Vision Disability Only page is for an eye doctor: an ophthalmologist, optometrist or neuro-ophthalmologist. It asks for corrected acuity, visual field measurements and how the vision loss affects the person's ability to use regular SEPTA service, so it should come from a clinician who has examined the patient's eyes.

Is the SEPTA vision page the same as the Functional Verification Form?

No. The Functional Verification Form is a separate, longer section of the ADA Application Packet in which a licensed professional describes the applicant's skills for using buses and light rail. The vision page is a focused eye report. Both are returned to SEPTA Access with the ADA application.
