NJ PIP Post-Service Appeal Form (Version 1.2)

Providers holding an assignment of benefits use this uniform form to appeal a New Jersey PIP insurer's payment decision on services already rendered and billed. Required for internal appeals since April 17, 2017 under N.J.A.C. 11:3-4.7B and Order A17-103, it replaced each insurer's own appeal form. The appeal must reach the insurer at least 45 days before arbitration or a Superior Court action.

Who completes it

The treating provider or billing office completes the whole form, including the appeal rationale narrative; for ambulatory surgery centers and other facilities, the responsible person who can certify its accuracy signs. A typed electronic signature is acceptable.

Submitting and payment

How to submit it
Send it to the insurer's internal appeals unit or PIP vendor as set out in the insurer's Decision Point Review Plan, with the original bill, the EOB and the narrative; the insurer has 30 days to respond.
Who pays
The appeal asks the PIP insurer to correct what it paid on the bill, citing reason codes such as an improper fee schedule amount, interest due on late payment or a retrospective medical necessity denial.

Good to know

  • Use one form per EOB. Choose bill-level codes (1 to 10) or line-level codes (A to S), not both, and list only the CPT, HCPCS or NDC codes being appealed.
  • Minimum attachments are the original bill (HCFA/UB), the EOB and an appeal rationale narrative; resend other records only if they are new or additional.
  • Code K covers retrospective medical necessity denials of care billed without an APTP in the first 10 days after the accident or before the first decision point.
  • At arbitration, either side may object to documents that were available but not submitted during the internal appeal, so include all supporting records that existed.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When can I go to arbitration after a New Jersey post-service appeal?

The appeal must be submitted at least 45 days before starting PIP dispute resolution under N.J.A.C. 11:3-5 or filing in Superior Court. Insurers have 30 days to respond, so even if a decision arrives on day 10, the provider waits until day 45 to file for arbitration.

Can one post-service appeal cover several bills?

No. DOBI's FAQ calls for one form per EOB. If the issues do not fit, use a second form completing only items 34 to 38 and note the extra page in item 29. Several forms may be sent together, each followed by its own supporting documents.

Can I e-sign the PIP Post-Service Appeal Form?

Yes. DOBI's FAQ says an electronic signature, including a typed name, is acceptable, and the signer certifies the information is true and correct either way. For facilities such as ambulatory surgery centers, the signer is the responsible person able to make that certification.
