Medical AI Scribe Tools: Templates, Workflow and Testing

VeroLauren Bennett · October 15, 2026 · 18 min read · Published by Vero Scribe Inc.

A medical AI scribe tool turns authorized encounter information into a draft for a clinician to review. The useful question is not simply whether it can produce a note. It is whether the chosen template preserves the right facts, uncertainty and unfinished work—and whether the reviewed version reaches the intended record.

Changing from SOAP to a brief progress note should change the organization, not the patient's story. Changing to a referral template is different: that output may require a referral decision and recipient that the source never supplied. A tool that fills every heading can be less useful than one that exposes missing information.

This guide focuses on template selection and repeatable output checks. For microphone and mobile-capture decisions, use the scribe app guide; for the broader task map, see AI scribe use cases.

Choose the output before the tool

Start with a sentence that names the work: “Prepare a progress-note draft from this encounter for the treating clinician.” Then name the authorized input, the person reviewing it and its destination. This prevents a useful text generator from being mistaken for an order-entry, referral-management or patient-messaging system.

Three common uses require different checks:

  • Encounter note: organize supplied history, findings, assessment and plan. A heading is not permission to generate an examination finding or diagnosis.
  • Patient-facing draft: translate the clinician's supplied explanation into suitable language. Preserve uncertainty and contact preferences; producing text is separate from approving and sending it.
  • Referral or handoff draft: carry the reason, relevant sources and outstanding work to a specified recipient. A missing decision or recipient is a blocking input, not a creative-writing opportunity.

“Scribe AI” also names a process-documentation product: it helps write SOPs, onboarding guides and other process documents using Scribe's step-by-step guides. That is a different task from preparing an encounter note. This article compares medical AI scribe tools, not business-process documentation software. Scribe AI overview and FAQs

Documented tools: inputs, templates and delivery

Compare the same controls using vendor documentation checked October 15, 2026. “Not established” identifies a question the reviewed documentation leaves open. Vero publishes this guide and develops one of the products compared.

Control

Custom instructions

Fixed headings, bracketed fill-in fields and parenthesized rules control structure and formatting. Edit the template directly or with Chat; Learn can turn reusable note edits into a template version.

Describe changes in natural language, edit the demo note's layout, or edit the Structure tab. Template versions can be restored.

Note-section templates accept fixed text, placeholders and conditional instructions. Users can apply them on demand or automatically at generation. These are section-level controls, not a universal whole-note template selector.

Shared templates

Owners choose Private, Team or Public. Team requires organization membership; Team and Public require a specialty. Copies saved by others remain when the original becomes private. Exact team-plan entitlement is not established by the sharing article.

Practice specifically includes team template sharing and template configuration support, alongside individual custom-template controls.

Dragon admin center distributes templates to organizational units and user accounts. Managing them requires a supported administrator role and a provisioned physicians environment.

Regeneration and manual edits

Updating a template does not rewrite an existing note. Request regeneration separately through Chat. The docs describe undo/redo, but do not establish that every manual correction survives regeneration.

Selecting a different template above an existing note regenerates the output. The template guide does not establish preservation of every manual note edit; compare the new output with the reviewed version.

Applying a template can regenerate a section while retaining edits. Adding another recording instead generates from the combined transcript without prior edits or style updates. Version history supports comparison and restoration.

Plan and access limits

Custom templates are listed across plans: Free shares ten actions/month; Pro has unlimited actions. Enterprise adds organization administration. Confirm which generation and revision actions consume the allowance.

Free custom outputs use the shared advanced-action allowance; Clinician and Practice list unlimited advanced templates. The new editor currently excludes Enterprise in its rollout notice.

These template controls are documented for the physicians product, not the nursing workflow. The reviewed sources do not establish a universal price or every partner's feature entitlement. Confirm the licensed implementation.

Input and delivery

Recording, written context or uploads feed the selected template. Review and edit the note, then copy to the EMR or use document exports.

Session transcripts feed note or document templates. Practice lists full EHR integration access; Clinician's Lite integrations depend on the EHR partner.

Recorded encounters produce drafts. Standalone content is transferred manually; embedded delivery and editing controls depend on the EHR partner's implementation.

Fit decision: shortlist a standalone workspace when your approved process includes manual transfer. Shortlist an embedded configuration when reducing that transfer is a requirement, then demonstrate its actual destination and permissions. For template-heavy work, test customization with the same source packet rather than comparing a standard note from one product with a heavily tuned note from another.

A template version is not a note version. Restoring a reusable format does not prove that a corrected note or saved EHR copy changed with it. Keep the reviewed note before rerunning ST-01, then check the deadline and voicemail preference in the regenerated output and receiving record. For Vero's capture route, stop an active recording before switching encounters. Vero workflow documentation

ST-01: one source, three template decisions

Fictional, editor-written documentation exercise. The source packet and sample drafts illustrate how the same facts fit different templates.

The fixed source packet

Use the same packet for every run. The test encounter is ST-01 / VISIT-01, October 15, 2026, 10:00.

  • F1 — patient report: “My right knee still bothers me going downstairs. I had an outside scan on October 8. They said they would send the report.”
  • F2 — available record: the outside report and images are absent from this packet. The patient has not supplied its findings.
  • F3 — clinician statement: “The cause is not established in today's supplied information. I have not performed a physical examination during this telephone encounter.”
  • F4 — supplied plan: the records coordinator will request the report today and check its receipt by October 16 at 15:00. If still absent, the coordinator will notify the covering clinician for a next-step decision. No request has yet been sent.
  • F5 — patient discussion: the patient agrees to a telephone update after clinician review and asks that no clinical details be left on voicemail.
  • F6 — output boundary: the clinician has supplied no referral decision, recipient, prescription or exercise recommendation. There is no examination or scan result to summarize.

F4 supplies this example's review deadline. Preserve that scenario-specific instruction and the uncertainty in F3 when changing templates.

Select the appropriate template

TemplateWhat may changeRequired decision
SOAP v1Separate patient report, available information, uncertainty and plan.Prepare a draft. Do not turn an empty objective section into normal findings.
Brief progress note v1Combine headings and remove repetition.Prepare a shorter draft while retaining F1–F6's relevant facts and boundaries.
Referral v1Would organize information for a receiving service.

Hold a send-ready referral: decision and recipient are absent. List missing inputs; do not invent them.

A complete-looking draft that fails

Patient reports right knee discomfort. Outside imaging is normal. Examination is normal. Physiotherapy referral sent. Patient will be contacted with results.

This version has five sentences and little clutter, but its apparent completeness is misleading. It invents findings and a completed action. It also drops the request owner, receipt deadline, escalation and voicemail restriction. “Patient will be contacted” does not preserve who first needs to obtain and review the missing report.

Show two acceptable drafts and the completed review

SOAP v1 — corrected teaching draft

S: Telephone encounter, October 15, 2026 at 10:00. Patient reports continuing right knee discomfort going downstairs and an outside scan on October 8.

O: Outside report and images are unavailable in the supplied record. No physical examination was performed during this telephone encounter.

A: Cause not established in the supplied information, per clinician.

P: Records coordinator to request the outside report today and check receipt by October 16 at 15:00; request not yet sent. If still absent, notify the covering clinician for a next-step decision. Patient agrees to a telephone update after clinician review; no clinical details on voicemail. No referral decision or prescription was supplied.

Brief progress note v1 — same facts, different structure

October 15, 2026, 10:00 telephone encounter: patient reports continuing right knee discomfort going downstairs and an outside scan on October 8. Report and images are unavailable; no physical examination performed. Clinician has not established a cause from the supplied information. Records coordinator to request the report today (not yet sent), check receipt by October 16 at 15:00 and notify the covering clinician if absent. Patient agrees to a telephone update after clinician review; no clinical details on voicemail. No referral decision or prescription supplied.

Completed fictional review record

Source: ST-01 F1–F6. Configuration: manually authored teaching outputs, SOAP v1 and Brief v1. Reviewer: example editor. Finding: flawed draft fails source fidelity and completeness. Remove the invented imaging, examination and referral claims; restore ownership, deadline, escalation and contact preference.

Recheck: both corrected drafts preserve the supplied information. Referral v1 remains held because F6 lacks authorization and recipient. Destination: teaching text only. Next step: clinician approval and a saved-record check in the chosen workflow.

Review facts retained and claims added separately

A completeness check asks whether the source facts survived. A support check asks whether every new assertion is justified. Run both: a draft can retain all six source items and still add “normal gait.” A draft can contain only accurate sentences and still omit the escalation instruction.

After editing, reopen the receiving encounter. Compare its text with the approved version and inspect any separate task queue. Copying the plan into a note does not assign the coordinator a task. If the workflow requires assignment, verify it independently, including owner, due time and acknowledgment route. The clinical notes quality guide develops these saved-record checks further.

Configure a template without adding facts

Write instructions about structure and handling, not prefilled clinical conclusions. A useful starter configuration for ST-01 is:

Use the supplied facts only. Attribute patient reports and dated outside information. Keep unavailable results unavailable. Include only clinician-supplied assessment and decisions. Preserve action status, owner, timing, escalation and communication preference. If a requested output needs missing authorization or facts, identify the missing input for the reviewer. Never populate normal findings from a heading alone.

Store a template identifier and version alongside each evaluation, then test the instructions against the source packet. If “brief” produces an omitted deadline, name that required field in the instructions and rerun the entire packet. Check the original defect and any new errors.

For Ontario physicians, CPSO's documentation policy addresses customizable templates and verification of template-populated entries. Check both defaults and free text against the source. CPSO Medical Records Documentation, section 5; checked October 15, 2026

Use separate versions for materially different purposes. A patient letter should not inherit internal shorthand or an unapproved instruction from a clinician note. A summary should not silently resolve conflicting medication histories. Define who can change shared templates, who approves a release and which saved examples must pass before it is used again.

Benefits and limits: what studies measured

Possible benefits include less first-draft typing, more consistent organization and faster preparation of several approved outputs. Their value depends on adoption, review effort and integration—not just generation speed.

In a randomized UCLA study, 238 outpatient physicians across 14 specialties received DAX Copilot, Nabla or usual care during November 2024–January 2025. Nabla reduced EHR time-in-note by 9.5% versus control (95% CI, 1.8%–17.2% reduction); the DAX estimate was a nonsignificant 1.7% reduction. The measure did not include editing inside the scribe applications. These are configuration- and study-specific findings, not total workflow time or demonstrated performance for Vero or every current product version. Lukac et al., randomized trial

A separate evaluation used 14 simulated ambulatory encounters across five platforms and examined errors including omissions, additions and partially correct information. Performance varied across and within platforms. That supports retaining the source packet and checking repeated outputs; it does not supply a universal accuracy percentage for a new template in a different clinic. Anderson et al., simulated-encounter study

The practical limitation is that a fluent note can be wrong in several ways at once. Recognition can lose a word; rewriting can alter certainty; a template can invite an unsupported finding; transfer can lose a corrected paragraph. Record the stage responsible instead of labeling every defect “transcription error.”

Privacy starts with the complete data route

Map audio → transcript/context → generated draft → reviewed note → receiving record, including uploads, logs, backups and exports. For each copy, identify access, location, retention, deletion process and responsible organization. Stopping capture, deleting a session and removing a downloaded PDF are separate events.

In U.S. HIPAA-regulated work, a cloud provider handling electronic protected health information on a covered entity's behalf can be a business associate even when it cannot decrypt the data. HHS explains the associated agreement and safeguard obligations; it does not certify or endorse specific products. Obtain the applicable BAA and assess the configured workflow rather than treating a marketing badge as approval. HHS cloud-computing guidance; checked October 15, 2026

Ontario organizations should use their own legal and professional requirements, including the IPC's January 2026 AI-scribe checklist. A U.S. agreement does not answer every Canadian data-handling question.

Before a clinical pilot, assign a privacy lead to approve the capture explanation, consent process where required, non-recording alternative and access controls. Test with fictional material first. Include a second speaker, an interruption and a request to stop capture; verify the documented device route rather than assuming screen lock or a closed tab stops recording. Do not place real patient details in a public bug report or template-sharing gallery.

Run a six-attempt template check

Trial plan (not yet run): run ST-01 twice through each of three templates—SOAP, brief progress and referral—for six attempts. Use the same source packet and save the exact configuration. Two runs per template provide a small regression check; broader reliability needs a larger evaluation.

  1. Fix the inputs. For an audio-capable tool, record the fictional packet once and retain the reference text. Document device, microphone, app/browser version, language, room conditions and speaker setup. Use an authorized replay/import route. If one product accepts only text, report that as a separate text-input configuration.
  2. Fix the templates. Save instructions, identifier and version. Expect SOAP and brief drafts to preserve F1–F6; expect the referral output to expose missing authorization and recipient rather than create a send-ready referral.
  3. Keep raw and corrected outputs. Do not silently replace a failed draft. Log unsupported claims, omitted required facts, altered action states and problems in the saved destination separately.
  4. Measure active effort. Record preparation, correction and transfer minutes by role. Keep elapsed generation latency separate from active work. An unavailable or aborted output is a failed attempt, not a zero-error note.
  5. Check the receiving record. Reopen the correct test encounter, compare its version and verify any task assignment. Record who approves the release and what remains unresolved.

Use a simple record for each attempt: run ID; tool/plan/version; source and template versions; date; input conditions; raw output reference; findings; corrections; active minutes; saved destination; reviewer; decision. Keep evidence in an approved private location. Publish only synthetic artifacts with permission.

Set acceptance rules before running: no invented findings or completed actions; all required source facts retained; missing referral authority exposed; approved content preserved after transfer. A failed check calls for correction and retesting. If a defect returns after a template change, retain both versions so the team can distinguish a configuration regression from different source material.

Free access and the cost of review

Separate an ongoing free entitlement from a paid trial, and a note allowance from unrestricted use of every feature. Heidi documents unlimited standard-template notes on Free, with ten shared advanced actions monthly; a custom-template note or document-template output consumes an action. Its 14-day paid-feature trial is separate. In the proposed six-attempt test, six chargeable custom generations would use six actions, leaving four only if no other advanced usage occurred. After the allowance, standard notes remain available while restricted actions wait for renewal or an upgrade. Confirm treatment of held or failed generations before a test. Heidi plan rules, checked October 15, 2026

Vero's published access includes a seven-day trial followed by a Free allowance of ten actions per month; Pro is a subscription and Enterprise is custom. Read the current price, billing interval, currency and included actions at checkout. Vero pricing, checked October 15, 2026

Hypothetical monthly budget in USD: 200 accepted notes, a $100 subscription, $60 allocated setup/support, three clinician review minutes per note valued at $120/hour, and one transfer minute per note valued at $30/hour. Total: $100 + $60 + $1,200 + $100 = $1,460, or $7.30 per accepted note. One extra review minute per note adds $400, raising the total to $1,860 or $9.30 per note.

Compare that total with the same work completed without the tool. Count failed attempts, training and recurring template maintenance; do not count both allocated setup and the same setup invoice twice. Time valued in dollars is an opportunity-cost estimate, not automatically a cash saving. The scribe pricing guide provides a broader purchasing framework.

The buying decision: choose the least complicated approved route that produces the required output and survives review and transfer. Keep a manual fallback. Recheck after changes to the template, source access, model, plan or receiving EHR—not just when the subscription renews.

Questions about choosing an AI scribe tool

Templates, inputs, integration and repeatable evaluation.

What is an AI scribe tool?

A clinical AI scribe prepares draft documentation from authorized encounter inputs. The clinician checks the facts, supplies missing clinical decisions and approves the record. Generating text is separate from signing a note, sending a message or executing an order.

Is the process-documentation product Scribe AI a medical scribe?

Scribe AI writes business-process documents such as SOPs and onboarding guides using Scribe guides. A medical AI scribe processes encounter information for clinical documentation. They serve different tasks; this article covers the medical category.

Which template should I start with?

Choose the template your receiving workflow actually needs. Run the same fictional source through that template and a shorter alternative. Prefer the structure that preserves required facts with the least correction, not the one that fills the most headings.

Should an empty examination heading become a normal examination?

No. A template heading does not establish that an examination occurred or what it showed. In ST-01, the source explicitly states that no physical examination was performed. Preserve that boundary instead of adding normal findings.

Can one encounter produce both a note and a referral?

Only when the supplied information supports both outputs. ST-01 supports a documentation draft but lacks a referral decision and recipient. A referral template should expose those missing inputs for the clinician rather than invent an authorized referral.

Will changing a template update an existing draft?

Establish the behavior for the exact product and version. Keep the reviewed draft before changing templates, inspect the new output, and reconcile manual corrections. Do not assume that an old draft, a regenerated draft and a saved EHR note remain synchronized.

Show all questions
How do I check whether a shorter note lost important facts?

Compare it against a source ledger rather than against the longer draft alone. In ST-01, receipt timing, escalation and the voicemail preference must survive compression. Then separately inspect the shorter note for unsupported new assertions.

Does an EHR integration mean the note is signed automatically?

Signing depends on the configured EHR workflow and approval permissions. Check where content lands, its draft or authenticated status, who can approve it and what the audit trail records. Demonstrate those steps in the chosen configuration.

Can a tool work with an interpreter?

Treat interpreter-mediated capture as a separate configuration. Verify supported languages, all audio channels and speaker attribution with qualified reviewers for each tested language. Keep interpreter statements, patient reports and clinician decisions distinguishable.

What device should I use for a trial?

Use a vendor-supported device and capture route approved by your organization. Record the app or browser version, microphone, operating system and audio conditions. Test phone and telehealth audio routes separately from text input.

Can a shared template be reused across specialties?

Reuse structure only after checking its assumptions and required fields. A specialty-specific default may introduce unsupported findings elsewhere. Give each approved variant a version, an owner and source-based examples that must pass after a change.

What should happen when a generation fails?

Retain the attempt in the log, use the approved fallback and record the extra work. Distinguish unavailable output from a completed output with no detected errors. Check whether retries consume the plan allowance before budgeting the evaluation.

How should I record an improvement after a template edit?

Keep the original source, old instructions and failed output. Rerun the full packet with the new template version, report which defect changed and inspect for new errors. Record the finding for that source, template and configuration.

Who owns the source recordings and exported copies?

Resolve ownership, access, retention and deletion for each copy in the applicable agreement and configuration. The clinic-side export and the vendor-held session may have different lifecycles. Deleting one is not evidence that the other was removed.

How do we decide whether to keep the tool?

Compare accepted outputs, active effort by role, failures and total costs against the same workflow without it. Confirm that required facts and permissions survive through the saved record. Continue only within the task and configuration the team actually evaluated.
