This guide helps clinical teams connect the official instrument to a clear result record. The copyable documentation template below is an original record scaffold, separate from the MoCA questionnaire. Worked examples connect the result, corrections and follow-up.
Sources and access requirements checked October 27, 2026.
How to access the official MoCA test PDF
MoCA Cognition manages access to the official forms. Its catalog distinguishes test families, versions and languages. Complete the provider’s applicable account and eligibility steps, and save the matching administration and scoring instructions with your service’s authorized clinical resources. Source: MoCA Cognition paper catalog, checked October 27, 2026.
Before using a downloaded file, check its title and version against the instructions. A filename such as “cognitive-test-final.pdf” does not tell the next staff member which instrument was administered. Give the clinical resource a recognizable label, record its source, and keep assessment records separate from the blank master copy.
For a patient or family member, the useful next step is to describe concerns about memory or thinking to the clinical team. Bring examples of changes in everyday activities and when they began. Avoid rehearsing test content before an assessment.
Current training, certification and permission requirements
As checked October 27, 2026, certification depends on the intended use. MoCA’s training page describes certification as optional when healthcare professionals use only the total score for screening, triage or referral, and required for interpreting individual tasks, subscores or cognitive domains. Its FAQ dates this policy change to December 2025. Training policy
| Intended activity | Check before use | Keep in the service record |
|---|---|---|
| Total-score screening, triage or referral | Current optional-certification policy; professional scope and employer requirements. | Purpose, authorized role and local competency expectations. |
| Task, subscore or cognitive-domain interpretation | Required certification and applicable language conditions. | Relevant training evidence and who is responsible for interpretation. |
| Research, redistribution or product incorporation | Separate authorization or licensing for the actual proposed use. | Approval reference, permitted materials, users and scope. |
The current terms specify the paper-based distinction, a language-availability exception, and recommended certification renewal every two years. They permit qualifying ordinary healthcare use without a separate commercial license, subject to conditions, but require separate authorization for research, redistribution, adaptations and incorporation into products or services. Non-commercial academic research still requires prior authorization. Terms, sections 5 and 10–11
Some older wording on the permissions page still describes a universal certification requirement. The current terms and training page describe the use-specific distinction above. Reproduction and distribution wording also differs across these pages; request clarification for the intended use. MoCA permissions policy
Publication and linking conditions
MoCA’s permissions page restricts publishing the test, instructions and links leading to them in newspaper or magazine articles, including electronic articles, and prohibits hosting the test or instructions on another website. For publication, redistribution or product integration, send the material, destination and intended audience through the official permission/contact route and retain the written response. Publication-permission clarification for this guide: pending.
Eligible students, faculty, academic researchers and public-institution employees may qualify for free training after verification. Confirm eligibility and the current fee through the official training service.
MoCA versions, languages and score denominators
A MoCA assessment PDF is not one interchangeable file. The official catalog includes the following families. Select through the official service and use that form’s instructions. Source: MoCA Cognition paper catalog, checked October 27, 2026.
| Form family | Published scale | Documentation distinction |
|---|---|---|
| Full | 30 points | Record the exact version; alternative versions exist. |
| Basic | 30 points | Designed for low education/literacy; retain the Basic label. |
| Hearing Impairment | 30 points | Uses visual flash cards; record the adapted form, version and language. |
| Blind/Telephone | 22 points | Voice-only form; retain its denominator and mode. |
| Five-minute Telephone | 15 points | A different brief form, not another name for the 22-point version. |
| Audiovisual | 30 points (Full via video) | Uses adapted videoconference materials and instructions; record remote administration. |
Choosing for hearing needs or a remote visit
Hearing difficulty: Ask the assessing clinician whether the Hearing Impairment form fits the person's needs, including access to its visual materials. Blind/Telephone relies on voice and is not a substitute for a hearing adaptation. Record the selected version, language and supports used.
Remote visit: For assessment using both video and audio, the official Audiovisual route provides modified Full-form instructions and materials. For voice-only contact, the catalog lists the distinct 22-point and 15-point telephone forms. Choose the appropriate route before administration; do not silently switch forms or denominators if video fails. Record the interruption and the assessor's decision about completion or reassessment. Source: MoCA Cognition FAQ, remote testing section, checked October 27, 2026.
If combined sensory needs or technical constraints make the intended form unsuitable, ask the responsible clinician to establish the assessment approach and record the decision. A shared 30-point scale does not make different forms interchangeable.
Record the assessment language separately from the note language. For example, an English clinical summary may describe an assessment administered in another language. Preserve the exact instrument used and any interpreter involvement or departure from its instructions. A convenient translation is not automatically an appropriate instrument for a particular person.
The official FAQ describes an education adjustment for MoCA Full, with different rules possible for other versions. Document the raw total, the assessor’s adjustment and the final total separately; consult the selected instructions for the actual rule. The same FAQ cautions that its described Blind-to-30 conversion has not been validated. Preserve the original result rather than silently relabeling it. From official form to a usable clinical record
A useful record answers three questions: what was administered, what was found, and what happens next? Build the handoff around those questions instead of a score alone.
- Identify the assessment. Record the exact instrument, version, language, mode, date and assessor. Link the completed source within the approved record system using the correct patient and encounter.
- Preserve the result. Capture completion status, raw total where supplied, adjustment and final score with denominator. If a source is unclear, assign clarification before a value becomes a trusted result.
- Add context and interpretation. Separate patient-reported concerns, collateral information, observed conditions and the clinician’s assessment. Name relevant limitations rather than inserting an automatic normal finding.
- Assign follow-up. Record the action, accepting person or team, agreed checkpoint and response if the task remains outstanding. A referral that has been sent still needs receipt and acceptance checks.
The medical history template can help organize history sources, while the referral letter guide shows the outgoing communication and internal tracking as separate records. Avoid duplicating a full sensitive history into every handoff when a concise clinical question and relevant findings will serve the recipient.
What a MoCA score can and cannot establish
The original 2005 validation study included 94 participants with mild cognitive impairment, 93 with mild Alzheimer’s disease and 90 healthy older controls, assessed at a community clinic and an academic center. At a cutoff of 26, the study reported 90% sensitivity for mild cognitive impairment and 87% specificity. These are results from that study population and design, not a probability that an individual patient has a particular diagnosis. Nasreddine and colleagues, original study
The Alzheimer’s Association’s DETeCD-ADRD primary-care guideline, published in December 2024, places cognitive testing within a broader evaluation: history and trajectory, daily function, information from the patient and an informant, and relevant clinical findings. The recommendations concern evaluation of suspected cognitive disorders; a brief score is one part of that process. Primary-care guideline
In the note, distinguish a screening result from a diagnostic conclusion. “MoCA Full final score 25/30; clinician plans further assessment in view of reported functional changes” communicates both the measurement and the decision. “Dementia confirmed by score” adds a conclusion the screening result cannot establish. Equally, a result above a screening threshold should not erase documented concerns that still need assessment.
Record concrete testing conditions when relevant: hearing or vision needs, language mismatch, interruptions, fatigue or acute changes. Describe what occurred and how the responsible clinician considered it. Avoid adjusting a score yourself to compensate for a difficulty without the applicable instructions and clinical review.
MO-01: a completed result and follow-up record
Fictional case: MO-01. Follow the source facts through the finished note and booking update.
Source packet
- Person and purpose: Rowan, aged 68, attends an outpatient visit on October 20, 2026, after reporting missed appointments over the preceding six months.
- Collateral source: With Rowan’s agreement, their adult child reports two late bill payments. The cause and effect on independent daily function need further assessment; this is not documented as established loss of independence.
- Assessment source: Assessor Lee records MoCA Full, English, version 8.1, in person, completed at 10:05. Lee records no interruptions and confirms the relevant instructions were followed. No additional examination findings are supplied.
- Result: The source worksheet records a raw total of 24/30, ten years of education reported by Rowan, an examiner-applied +1 education adjustment, and a final total of 25/30. The example’s adjustment is already included in that final score.
- Clinician decision: Clinician Morgan documents further cognitive and functional assessment, including medication and medical-history review. No etiological diagnosis is made from this screening result.
- Agreed next steps: Rowan agrees to follow-up. Morgan accepts responsibility for the clinical review on November 3. Coordinator Casey accepts booking and receipt-check tasks for October 23, with the duty coordinator covering absence. If booking remains unresolved, Casey contacts Morgan that day for an alternative plan and updates Rowan using the agreed phone route.
The supplied +1 adjustment follows the official FAQ’s education rule for MoCA Full, checked October 27, 2026.
Finished results note
October 20, 2026 — cognitive screening and follow-up. Rowan reports missed appointments over six months. With Rowan’s agreement, their adult child reports two late bill payments. Their relationship to cognition and effect on independent function remain to be assessed. Lee administered MoCA Full, English v8.1, in person at 10:05; completed, with no interruptions recorded. Source worksheet: raw 24/30; examiner-applied education adjustment +1 based on ten reported years of education; final 25/30, adjustment included. Morgan reviewed the result alongside the reported concerns and plans further cognitive/functional assessment and medication/medical-history review. No etiological diagnosis is assigned from the screening score. Rowan agrees to follow-up. Morgan owns the November 3 clinical review. Casey owns booking/receipt confirmation by October 23; duty coordinator covers absence. If unresolved, contact Morgan that day and communicate the revised plan to Rowan by the agreed phone route. Follow-up assessment remains pending.
The note preserves the education source and distinguishes reported bill-payment problems from a clinician-established functional impairment. The clinical-note examples explain how to keep each statement connected to its source.
Close the booking loop without closing the clinical task
At 14:10 on October 23, Casey confirms the November 3 appointment in the receiving schedule. Morgan remains the named reviewing clinician. Casey tells Rowan by the agreed phone route and records confirmation of the plan. The booking task is complete; the assessment task remains open. If Rowan does not attend, the service’s follow-up process requires a new recorded action and owner. A completed scheduling checkbox cannot stand in for a completed clinical review.
MO-02: correcting a score without losing the follow-up
In this fictional continuation of MO-01, the result is transferred into a system with a field labeled only “MoCA score.” A staff member enters the final 25, then another workflow adds the education point again. The saved result becomes 26/30 while the source remains 25/30. Both records refer to the same assessment.
| Record | Before correction | Verified correction |
|---|---|---|
| Source worksheet | Raw 24; adjustment +1; final 25/30. | Source retained; no new assessment created. |
| Structured result | 26/30 after the adjustment is applied twice. | 25/30 final; adjustment already included. Correction author, time and source recorded. |
| Narrative | Auto-text calls the entered score “normal.” | Unsupported auto-text removed through the correction process; Morgan’s interpretation retained. |
| Outgoing communication | A referral contains the erroneous 26/30. | Corrected communication sent; receiving team acknowledges the replacement result. |
| Follow-up task | Risk of closing work because the entered score crossed a threshold. | Morgan confirms the further-assessment plan remains. Casey checks that the booking task and clinical task have the right statuses. |
Record the correction at the time it is made. Retain the previous value and reason in the system’s correction history rather than silently overwriting the past. Reopen the structured result and the saved narrative; verify the corrected value also reached any intended recipient. The medical records guide develops the relationship between an amendment, an exported copy and the history needed to reconstruct the change.
Practice check: which record still needs attention?
The structured score and note now both say 25/30. The receiving team has acknowledged the correction, but the local task list says “cognitive assessment complete.” That task still needs correction: only screening and booking have occurred. Confirm the pending assessment’s owner, checkpoint and status. Accurate arithmetic does not prove the follow-up happened.
Telephone and incomplete assessments: preserve the source
Consider a separate fictional referral that supplies MoCA Blind/Telephone, 17/22, with its date and assessor. The receiving EHR offers only a generic 30-point field. A staff member calculates a percentage and stores “23/30.” That creates a new derived value without preserving how it arose.
Keep 17/22 and the exact form in the result record. If the receiving field cannot represent that denominator, use the approved alternative record location and flag the mapping problem for the responsible team. Ask the interpreting clinician to review the original result and its context. Do not use a guessed full-form value to attach a diagnostic label. This is particularly important because the official FAQ labels its own proposed conversion as unvalidated.
For an interrupted assessment, separate not completed, not scorable under the applicable instructions, and completed with a limitation. These are different statements. Record which applies, who made that determination, the reason and the next action. If the assessor has not supplied a valid total, the record should show an unresolved result rather than a zero or an automatically completed normal finding.
For later comparisons, keep each result’s date, form, language, administration mode and relevant conditions visible. A difference in two numbers may also reflect a different measurement context. Explain what the clinician considers comparable before describing improvement or deterioration.
Copyable MoCA results documentation template
Use these blank prompts to record the assessment source, result and follow-up. Complete the fields from the assessor’s record and clinician’s decisions. Assign a clarification owner for unavailable information.
A short final note may combine several fields into a paragraph, as MO-01 does. Keep the underlying distinctions inspectable. For example, “25/30” is less useful than “raw 24/30; +1 documented adjustment; final 25/30.” Likewise, “referred” is less useful than a named destination, sending date, acceptance status and next checkpoint.
Quality check before saving or sending
Check the result against the source and the receiving record, not only against the draft on screen:
- Identity: Correct patient, encounter, assessment date and assessor.
- Instrument: Exact form family, version, language and mode retained.
- Score: Correct denominator; raw and adjusted values distinguished; no duplicated adjustment or silent conversion.
- Completion: Missing, interrupted or uncertain information remains visible.
- Context: Patient reports, collateral information and clinical findings have distinct sources.
- Interpretation: The responsible clinician’s reasoning is recorded; a threshold has not generated an unsupported diagnosis or severity label.
- Action: Next step, accepting owner, checkpoint and unresolved-task response are recorded.
- Destination: The saved structured field, narrative, attachment reference and outgoing communication agree.
Limit access to completed assessments through the organization’s approved record and sharing processes. Keep blank resource management separate from identifiable clinical records. For a referral, include the relevant clinical context and result through the approved channel; track both delivery and the recipient’s next responsibility.
If an approved drafting tool helps prepare the note, give it the verified result and decisions. Review the final saved document for substituted denominators, omitted limitations and tasks falsely described as complete. The medical documentation workflow provides a broader framework for that final review. The purpose of this record is to let the next clinician understand the assessment that actually happened and the care still to be arranged.
Clinical review: pending.
Questions about MoCA PDF access and result records
Official forms, training requirements, version checks and follow-up documentation.
Where can I find the official Montreal Cognitive Assessment PDF?
MoCA Cognition distributes the official forms through its access process. Complete the applicable account and eligibility steps, then select the form family, version and language with its matching instructions. Form-download links are withheld here while publication-permission clarification is pending.
Is the results template here the MoCA test?
It is an original Vero record template for an assessment already performed. Its fields capture the instrument, result, interpretation and follow-up. Obtain the questionnaire and scoring instructions through MoCA Cognition.
Is MoCA certification still mandatory?
The current policy distinguishes total-score screening, triage or referral from task-level interpretation. Certification is optional for the former and required for the latter, subject to the conditions explained in the training section. Check your role and institution’s requirements as well.
Can a patient use the PDF to diagnose themselves?
A MoCA result belongs within professional assessment. A patient concerned about memory or thinking can discuss specific changes and their effect on daily activities with a clinician; a self-calculated score does not establish a diagnosis.
Can our clinic put the MoCA PDF on its website?
MoCA’s permissions page prohibits website hosting of the test and instructions and restricts links in electronic articles. Request written clarification for the proposed material, destination and audience through MoCA Cognition.
Which language should be recorded?
Record the exact language of the instrument administered, alongside the person’s language background and any relevant communication support. Keep the language field separate from the language in which the clinical note is written.
Show all questions
Are all MoCA forms scored out of 30?
No. The official catalog lists Full and Basic on 30-point scales, Blind/Telephone on a 22-point scale and the five-minute Telephone form on a 15-point scale. Preserve the actual form and denominator.
How should an education adjustment appear in the note?
Keep the assessor’s raw total, the adjustment and the final total in separate fields. State which instructions were followed. The final score must make clear whether the adjustment is already included, as MO-01 demonstrates.
Does a result below 26 diagnose dementia?
No. A screening threshold is not a diagnosis or a disease-severity label. Interpretation also considers history, function, context and other assessment findings. Record the clinician’s conclusion separately from the numeric result.
Can a telephone result be converted to a full-test score?
Retain the original result and denominator. MoCA’s FAQ notes that its described Blind-to-30 conversion has not been validated. A calculated proportion should not silently replace the original test result or acquire a different form’s interpretation.
What if the assessment was interrupted?
Record completion status, the reason and the assessor’s scoring decision. If a valid total has not been supplied, leave the total unresolved and assign clarification. Avoid completing missing observations from a prior note.
How often should the MoCA be repeated?
Record the clinician’s purpose and planned interval for that person. For comparisons, preserve previous dates, versions and testing conditions, and consult the official FAQ on alternate forms and possible learning effects.
What belongs in a referral accompanying the result?
Include the clinical question, exact instrument and result, relevant concerns and functional history, conditions affecting interpretation, and the requested next step. Track sending, receipt and acceptance separately.
What should happen after a corrected score is entered?
Recheck the structured result, narrative and any already-sent communication. Preserve the correction’s author, time and source, notify affected recipients, and ask the responsible clinician whether the follow-up plan changes.
Can an AI scribe document a MoCA result?
It can draft from results and decisions supplied through an approved workflow. Review the instrument label, denominator, adjustment, completion state and follow-up task. A drafting tool cannot supply missing assessment observations or make an unrecorded interpretation.
